The celebrant's first and last name
Phone number.
Full birthdate (month, day, and year)
A short bio such as how they came to know See's Candies or their favorite candies.
Must include an address for delivery (no P.O. boxes permitted)
Requests usually take between 2 and 4 weeks from when the request was received and processed. During the high shipping volume season (winter), See's puts a pause on shipping until the new year.
The nominee will be placed on a waiting list.
